This page explains what information Apex Medical Transportation collects through apexnemt.com and how we use it.
What we collect
- Ride requests: the name, phone and email you give us and your relationship to the rider; the rider's name, date of birth, gender, height and weight; pickup and drop-off addresses, date and time; what the rider needs for the ride (mobility, steps, wheelchair, portable oxygen, anyone riding along); any notes you add; and how the ride will be paid for, including the facility and billing contact if a facility is paying.
- Facility inquiries: facility name and type, your name, role and contact details, expected ride volume, and your message.
- Job applications: your name, contact details and résumé.
- Messages: anything you send through the contact form.
- Technical information: your IP address and browser details, which we use to keep the site secure and block spam.
Health information
The ride request form asks only what we need to send the right vehicle and crew, such as mobility, height and weight, and portable oxygen. Please don't send diagnoses or medical history through this site.
How we use it
To respond to your request, schedule and confirm rides, review job applications, follow up on facility inquiries, and keep this site running.
Who we share it with
We use service providers to host this website and send email. Some parts of the site also use outside services:
- Google: the address suggestions on the ride request form send what you type to Google Maps, and the site's fonts load from Google Fonts.
- Cloudflare: our forms use Cloudflare Turnstile to check that a person, not a bot, is sending them.
We don't sell the information you send us.
